Job Summary

An exciting opportunity has become available for a Specialist Nurse with the Health Inclusion Team for 28.5 hrs. We are looking for a strong team player, who can work on their own initiative and provide a person-centred approach to patients independence and well-being. KEY RESULT AREAS:

To support UK new entrants, register with their preferred choice of local GP

Main duties of the job

The health inclusion team aims to support individuals who are socially excluded, who typically experience multiple interacting risk factors for poor health. This includes facilitating the integration of overseas arrivals into mainstream health services and address inequalities with health and wellbeing within the borough of Barnsley.

The Health Inclusion Team is part of the local outreach team working in partnership with Barnsley Councils Housing Service and local substance misuse service, working directly on the streets to locate, engage with and support individuals that are homeless, have been homeless or are at risk of becoming homeless.

The service also provides a weekly wound care drop-in clinic for individuals that are living street-based lifestyles, run in collaboration with Waythrough's Barnsley Recovery Steps service. At the clinic wounds are assessed with basic treatment and self-care advice provided.

COMMUNICATION WITH OTHERS:

Health Inclusion Team provide face to face support at the womens migration group, community halls of Roma gypsy travellers sites to provide a cohesive and collaborative effective service.

Attend MDT and Safeguarding meetings as a professional advocate for the trust and patients we care for.

Attend the Homeless and Inclusion Health Network

Job responsibilities

JOB SUMMARY

To work with the HIT Lead Nurse and other team members to provide high quality holistic health needs assessments, which is culturally sensitive to ethnic minorities and other marginalised client groups e.g. asylum seekers, homeless, gypsy and travellers residing within the borough of Barnsley.

To offer health assessments/screening to clients within this group; incorporating TB and other communicable disease screening as appropriate.

Signposting clients to appropriate services.

Clients will be supported to access and integrate with mainstream health and other services.

To act as a resource for health and other statutory/voluntary/agencies.

To promote awareness and good practice in the delivery of health care for the client group.

Core Duties And Responsibilities:

To work as a member of the Health Inclusion Team, developing partnership working and providing specialist knowledge in this field. To organise and be responsible for management of corporate workload, delegating and providing assistance to others as required.

To be responsible for the management of a defined client group, within the team.

Using a holistic approach, provide a baseline and initial assessment for clients, in accordance with nursing protocols; taking into account cultural background.

To provide tuberculosis screening, treatment and contact tracing as required in line with Barnsley and National TB screening policy and guidelines.

To identify other screening or therapeutic interventions, signposting to appropriate services as required, ensuring clients health needs are addressed.

Interventions to be undertaken in accordance with procedures, policies and protocols; incorporating matters of a sensitive nature; which may include previous trauma or torture, imprisonment, rape, HIV, sexual or environmental health issues, safeguarding children, vulnerable adults, mental health issues, communicable and notifiable disease including TB. Refer to other services as appropriate.

To facilitate access to health services, including assistance with registration at GP practices, as required.

Undertake any health promotion activities relevant to this client group, in association with the Health Protection Agency, Public Health Department; for example flu vaccinations.

To act as a specialist resource to provide education and advice on health provision in relation to this client group, both locally and nationally, to all agencies involved; including other health care professionals and students providing an appropriate learning environment.

To utilise interpretation services effectively in accordance with policy; reporting any inefficiencies.

Adhere to and as appropriate contribute to the development and review of policies, procedures and standards relating to the service; using an evidence based approach.

Work within and as appropriate contribute to the clinical governance framework to maintain consistently high standards of care, working from an evidence base or current best practice.

Contribute to the audit process undertaken within the service, in order to provide evidence of effectiveness, efficiency and highlight areas for development.

Effectively manage members of the team, delegating work appropriately to ensure the best use of skills and resources, supporting professional development and undertaking appraisals.

Undertake Nurse Prescribing in response to assessed need and in accordance with trust policy.

Ensure complete, accurate and up to date records are maintained in accordance with NMC guidelines and trust policy. Record all client contacts and activities, and input data into appropriate IT system in a timely manner in accordance with PCT protocols.

Adhere to the NMC scope of professional practice and trust policies and procedures; ensuring professional competency and development by updating skills and knowledge; attending all mandatory training.

Communication With Others:

Communicate effectively, empathically and in a cultural sensitive manner with clients, representatives, carers and their families, interpreters will be used effectively where the need is identified.

Communicate effectively with all members of the Health Inclusion Team; providing information as necessary to provide a cohesive and effective service.

Communicate effectively with other health professionals, statutory and voluntary agencies to ensure a team approach to client care; providing explanations of any relevant cultural issues as required.
