Exciting new opportunity for a Senior Community Nurse Lead role within Community Service.

We are looking for a compassionate, hardworking and motivated Senior Leader to join our Community nursing team in the South East Essex area on a permanent basis at Band 7. This is an opportunity for a developing leader to gain experience and enhance their skills.

You will have responsibility for managing & leading the community nurses. In addition you will support the team in Triaging calls/referrals and providing unplanned care to patients within the community setting. You will liaise closely with other members of the wider MDT as well as with patients and their families, therefore excellent communication and IT skills are essential.

To provide clinical leadership within the South East community nursing service. To provide specialist advice in community nursing and to be the interface with primary and acute care. To hold close working relationships with partners and to engage in local system transformations.

Main duties of the job

  • To provide expert senior leadership within the team. To work autonomously within the guidelines and the policies and procedures of the Trust.
  • Provide leadership advice and support to colleagues as per Manager's Handbook.
  • To work in conjunction with other Team Managers/Leaders and Head of Community Nursing ensuring that all services work collaboratively to provide best possible quality of care to patients 24/7.
  • Lead the team in the evaluation and monitoring of the service in conjunction with Head of Community Nursing, including audits, projects, benchmarking, safer staffing, transformational work, etc.
