An exciting opportunity has arisen for a well-motivated, enthusiastic physiotherapist to work for South West Yorkshire Partnership NHS Trust within the cardiac and pulmonary rehabilitation service.

This community-based team provides quality care and support for patients undertaking either the cardiac or pulmonary rehabilitation programmes.

The service is responsible for delivering a specialist care to patients in their own home as well in our community venues around Barnsley. The successful candidate will be expected to assess and treat patients with a wide variety of cardiac and respiratory conditions.

Main duties of the job

Enthusiasm and the ability to use your own initiative are essential, along with good written and verbal communication skills and the ability to manage your time effectively.

You will need the ability to work both as part of a team and as a lone worker, along with the use of a vehicle to undertake any necessary travel in connection with the duties of the post.

You will be required to manage your own caseload and contribute to the daily management of incoming group tasks. Daily duties include triaging referrals, undertaking initial assessments, including exercise testing, monitoring and progressing patients during exercise, discharge assessments, delivering education sessions and recording electronic patient notes.

It would also be advantageous to have a good understanding of cardiac and pulmonary conditions such as heart attack, cardiac surgery and COPD.

You will be allocated to a specific venue; however, it should be noted that you are employed to work across the service and will be required to move bases throughout the working week.

Job responsibilities

JOB SUMMARY

This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed.

The content of this post will be reviewed in consultation with the postholder when necessary and in line with the service developments.

JOB PURPOSE:

  • To work within the Cardiac/Pulmonary Rehabilitation Team to provide physiotherapy expertise within the multi-disciplinary team.
  • To assess, treat and advise own caseload of patients with supervision from a senior member of staff when necessary and formulate a clinical diagnosis and treatment plan as an autonomous practitioner.

CORE DUTIES AND RESPONSIBILITIES:

  • To undertake an assessment of patients using clinical reasoning skills and assessment techniques.
  • Formulate and deliver individual/group work physiotherapy treatment programmes based on sound knowledge of evidence based practice and treatment options.
  • To interpret and analyse clinical and non-clinical information to form a diagnosis, prognosis and recommend best course of intervention, developing comprehensive discharge plans supported by senior physiotherapists.
  • To ensure HCPC registration is maintained
  • To communicate effectively with patients and carers to progress rehabilitation and treatment programmes.
  • Evaluate patient progress, reassess and alter treatment programmes if required.
  • To provide advice to medical, nursing staff and other members of the multi- disciplinary team regarding patients treatment & progress.
  • To be responsible for risk assessment of own caseload & working area.
  • To be responsible for competent and safe use of all equipment, patient appliances and aids.
  • Record all patient contacts and activities accurately and contemporaneously maintaining accurate records and statistical returns as required by the Trust including entering data onto the Trusts systems within the required timeframes.
  • Advise patients on health promotion issues in order to achieve pre-determined local and national health targets.
  • Participate in health screening programmes and disease management of patients under the direction of the Specialist Nurse / Physiotherapist / Clinical Team Leader.
  • To be a skilled communicator establishing and maintaining excellent relationships with members of the cardiac / pulmonary rehabilitation team and other Health and Social Care professionals, initiating referrals where appropriate to ensure adequate support for patients and carers.
  • To be responsible for maintaining own competency to practice through CPD activities and maintain a portfolio which reflects personal development.
  • Maintain and develop current knowledge of evidence-based practice & develop specialist knowledge within CVR area.
  • To be responsible for maintaining accurate and comprehensive patient treatment records in line with Chartered Society of Physiotherapy (CSP) and Trust standards.
  • To support Physiotherapy Clinical Supervisors in the supervision and co-ordination of students and assistants when necessary.

COMMUNICATION WITH OTHERS:

  • Patients, their carers and families.
  • All members of the cardiac / pulmonary rehabilitation team in order to maintain professional relationships and sharing of information.
  • Colleagues within the larger team to provide workload cover and professional support.
  • Other health professionals (hospital and community), and statutory and voluntary agencies to ensure a team approach to client care.
