Macmillan Clinical Nurse Specialist (CNS) in Palliative Care
Requirements
Candidates must be a 1st Level Registered Nurse with a degree and a diploma in Specialist Palliative Care, along with experience in palliative care settings. Independent non-medical prescribing and mentoring skills are essential, with a preference for additional qualifications in counseling or oncology.

An exciting opportunity has now arisen within the Community Specialist Palliative Care Team for a Clinical Nurse Specialist in Palliative Care.
In this role, you will work across the Barnsley neighbourhoods, providing supportive palliative care to enhance the quality of life for patients/carers with any life limiting illness requiring palliative care, including complex physical, psychological & emotional needs.
The post holder will possess extensive knowledge of palliative medication to support the complex symptom management needs of the patient.
As Band 7 Macmillan CNS Palliative Care, you will be able to demonstrate:
ability to manage a caseload of people with specialist palliative care needs
excellent working knowledge of pain & symptom management & managing complex emotional & psychological needs
ability to provide highly specialised advice/guidance to members of the wider health & social care team
ability to design & deliver palliative care education
ability to work as an effective team member

Main duties of the job
As Macmillan Clinical Nurse Specialist Palliative Care, You Will:
Assess, develop & implement specialist palliative care for patients & family, monitoring & evaluating the effectiveness of that care enabling patients to be maintained in the environment of their choice
Work closely with neighbourhood teams, primary care, & adult social care partners to ensure seamless support for individuals with complex needs
Be equipped with the knowledge, understanding, skills, attitudes & behaviours relevant to employment & act in accordance with the NMC Code Professional standards of practice
Have a key role in training, education & staff development, actively participate in regular supervision & appraisals contributing to clinical leadership facilitating a culture of learning/reflective practice
Have high level communication & interpersonal skills

JOB SUMMARY
Macmillan Clinical Nurse Specialists Palliative Care provide expert advice, information, education and support to adult patients, and those important to them, with advanced and progressive disease who have complex palliative and end of life care needs, and to professional colleagues.
Macmillan Clinical Nurse Specialists Palliative Care deliver personalised care following holistic assessment to support the management of symptoms and meet changing physical, social, psychological and spiritual needs.
Macmillan Clinical Nurse Specialists Palliative Care are competent and up to date in their expert knowledge and practice to support the delivery of palliative and end of life care in their specialist role.
Provide and lead specialist education to others to promote best practice in Palliative Care.

The postholder will:
1.1Care/service/function Provision:
To manage own specific geographical caseload, and day to day management of colleagues in absence of team members.
To give highly specialised advice, information, guidance and support to patients, families, Neighbourhood teams, Primary Health Care Team, Allied Health Care Professionals and Social Care Professionals.
To provide highly specialist clinical leadership for patients and their families to meet individual palliative care needs in the community, working independently with significant discretion.
To autonomously assess, plan, implement and evaluate the care for individual patients and their families to meet individual palliative care needs in the community, working independently with significant discretion.
To function as an independent non-medical prescriber and carry out relevant CPD to maintain competence.
To develop effective and empathetic supportive relationships with patients and their families, using advanced communication skills to relate highly complex, contentious, sensitive information taking into account barriers to understanding and challenging communication.
To monitor and identify complex bereavement issues and refer to appropriate services.
To act as a resource in developing and signposting information for patients and staff.
1.2 Working As Part Of a Team:
To act as Specialist Palliative Care representative at primary care Multi-Disciplinary Team Meetings.
To be a core member of Specialist Palliative Care Multi-Disciplinary Team meeting and neighbourhood team.
To initiate, design and deliver palliative care education programmes for multi professional groups to promote high quality palliative care.
To act as a mentor to newly appointed clinical nurse specialists in palliative care setting
To contribute to business planning for service development and strategic planning in palliative care services
To develop network and regional contacts enabling the sharing and promotion of new developments in Palliative Care.
To participate in Team responsibilities for efficient management of Palliative Care Services, including the accepting, recording and safe keeping of donations from the general public for Macmillan and Home Care Support Fund.
To participate in the formulation and implementation of policies and procedures and evaluate standards of care within palliative care services.
To develop and lead integrated audit systems, which influence and guide palliative care practice in line with Department of Health Guidelines.
To participate in national research projects.
To maintain accurate manual and computerised records to ensure continuity of care and facilitate accurate data collection for service development
1.3 Managing Self:
To critically evaluate and disseminate existing research to promote evidence-based practice incorporating into SWYPFT Clinical Governance Guidelines.
To participate in professional development and clinical supervision programmes.
To be aware of and act in accordance with Trust clinical standards, guidelines, Health and Safety and work within the NMC Code of Conduct.
To have developed flexible self-management skills and have the ability to concentrate where the work pattern is unpredictable.
1.4 Management Supervision and Support
To be part of the core neighbourhood team and provide leadership for palliative/ end of life care team for the team. This includes provision of supervision and clinical advice and direction.
