Early Help Coordinator - Open to Review

How you'll make a difference

As a Social Work Assistant, you will be working directly with disabled children and young people, their families and wider support networks, providing practical support to promote inclusion, independence and positive outcomes. 

Working as part of the wider 0-25 Social Care Service within the 0-18 team, you will support children and young people to reach their full potential wile working collaboratively with colleagues, families and partner agencies.

What you will be doing

• It will be your responsibility to organise and chair review meetings, prepare and share reports, and represent the team and the children and families you work with at a range of multi-agency meetings.  
• Visiting children and young people in a variety of settings, including their homes and schools, you will build positive relationships and develop a strong understanding of their lived experiences.
• Working closely with agencies, providers and family members, you will help ensure that children and families receive the right level of coordinated support to meet their needs. 
• You will review plans created by social workers within the team, making sure they are progressing effectively and achieving positive outcomes for children and families. 
• Gathering and sharing information to inform reviews, assessments and plans, you will ensure the voice, views and needs of the child or young person remain at the centre of the decision-making and practice.
• You will contribute towards Education, Health and Care Plans (EHCP's), working collaboratively with professionals to support the best possible outcomes for children and young people. 

What we need from you

• It is essential that you hold an NVQ Level 3, or equivalent, or be working towards an appropriate qualification related to the provision of services for children and young people.
• You will have experience of working with children, young people and families in a statutory setting.
• We are looking for someone who has experience of risk assessment and risk management, with the ability to identify concerns and take appropriate action.
• You must have experience or knowledge of relevant childcare legislation, including child protection, regulations and codes of practice, processes and procedures.

What you need to know

• This role is for a limited term of 12 months, working 18.5 hours per day, one of which must be a Wednesday for 7.4 hours. The remaining hours worked can be agreed at interview.
• Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post for both Children and Adults.
• You will be required to travel widely within the authority so you must have a full, valid driving licence and provide a car or have access to an appropriate means of travel.
• Business insurance will need to be added to your current car insurance policy.
