Requirements

Candidates must be HCPC registered and educated to degree level in Physiotherapy, with experience in surgical and medical inpatients. Experience in multidisciplinary teamwork and discharge planning is essential, along with the ability to supervise and train other staff.

Job Summary

The post holder will provide a comprehensive physiotherapy service to patients on the medical and surgical wards who are presenting with complex rehabilitation needs to enhance the care provided within the routine caseloads.

The post sits within the acute Physiotherapy Service who provide Physiotherapy cover to all the medical and surgical wards to facilitate early discharge planning in GWH across a seven day service. This will include complex, multipathologies & neurological conditions.

The postholder mustbe aware of the total Physiotherapy needs of the Acute Physiotherapy Team, the changing demands on the service, and take action in consultation with the Team Leaders to fulfil these needs (including reprioritisation of the complex caseload). The postholder will ensure data is collected to allow review of the efficacy of the post, and how it improves patient care over the term of the post.

Main duties of the job


  • To be responsible for managing own caseload of complex patients across multiple wards ensuring evidence based interventions are completed in the acute setting, and early discharge planning and referrals for ongoing interventions are completed. This is to ensure timely discharges and minimal therapy delays from this setting.
  • To clinically support the rehabilitation team across all wards as needed, including assessing, treating and managing complex cases as required.
  • To set appropriate therapeutic goals based on accurate prognosis for patients to monitor outcomes, evaluate patient progress and adjust treatment required to ensure effective discharge.
  • To utilise the Acute Therapies prioritisation system and work flexibly when needed to meet demand across the Acute Therapies services.
  • To chair team meetings, plan staffing, carry out the daily coordination role, and support with managing workloads.

Qualifications Essential


  • HCPC Registered
  • Educated to Degree level in Physiotherapy or equivalent training and experience
  • Professional/ clinical knowledge acquired through degree supplemented by specialist training experience in specialist fields to post graduate diploma level or equivalent


Experience Essential


  • Experience working in surgical / medical inpatients, respiratory and rehabilitation
  • Experience of working as part of a MDT and of discharge planning
  • Experience of working on a respiratory on-call / weekend rota
  • Experience of supervising/managing students on placement
  • Supervision and training of other staff
  • Participation in service audit


Desirable


  • Experience in orthopaedics, front door, community and rapid response/admission avoidance
  • Experience of using outcome measures
  • Participation in service development and implementation of change
