Are you looking for an exciting opportunity? We are looking for an experienced Health Care Support Worker to join the Kirklees Learning Disabilities Community Health Team.
The team is multi-disciplinary and provides specialist health support to both individuals, and their wider support networks. The team work agilely and covers the Kirklees locality. The work is varied and will give the right candidate the chance to develop a range of skills working alongside a team that includes nursing, occupational therapy, speech & language therapy, physiotherapy, psychology, and psychiatry.

Main duties of the job
Will carry out to a high standard (Under the supervision/direction of the registered Nurse) specific tasks/planned programmes of care with a designated group of service users and be able to assess, problem solve or report, in a timely and proactive manner, when these standards have not been met.
Postholder is expected to demonstrate an understanding of the care and treatment programmes and be able to make the appropriate adjustments independently within own level of competence and be able to communicate any changes to service users and team members.
Will be supported to develop and maintain meaningful therapeutic relationships with people. Adopt a polite, positive, and empathetic approach, communicating effectively and informatively.
You are expected to be able to prioritise own tasks, exercising initiative and judgement in response to peoples needs. Refer and seek guidance/supervision appropriately and timely.

Undertake the physical aspects of care eg, moving and handling of service users. Use items of equipment in delivering care eg, hoists, wheelchairs etc. (as appropriate to the role), ensuring appropriate instruction on their use has been received.
assist in dealing with verbal/physical aggression using de-escalation techniques, this may include the need for restraint and/or seclusion (as appropriate to the role).
Support service users with personal care duties (bathing, toileting, dressing, support with meals, assist patients with their appearance), as part of a healthcare team, provide the highest standards of safe and compassionate planned care to people that maximises their recovery and rehabilitation potential.
Undertakes a limited range of delegated clinical care duties relevant to the service provided/work area and records patient observations: e.g. taking blood pressure, blood glucose monitoring, pregnancy testing, observing patients for signs of agitation or distress, removal of peripheral cannula urinalysis, removal of catheter.
participate in a flexible rostering arrangement providing 24-hour/7 day care (as appropriate to the role).
