Job Description

The Role

An exciting opportunity has arisen for motivated and enthusiastic Advanced Frailty Practitioners to join the Home First Response Service (HFRS) within NHS Greater Glasgow and Clyde.

You will support the delivery of person-centred, frailty-focused care across the acute and community interface, working closely with Emergency Departments, Acute Assessment Areas, Hospital at Home, community teams and wider multidisciplinary partners. The service aims to prevent unnecessary hospital admissions, facilitate early discharge and support patients to remain at home wherever possible.

This role provides an opportunity to work alongside experienced practitioners and contribute to innovative models of care that improve outcomes for frail older adults.

NHS Greater Glasgow and Clyde (NHSGGC) may be able to provide visa sponsorship for certain roles, subject to the post meeting the relevant eligibility criteria. Sponsorship is not guaranteed and will be determined on a case-by-case basis following interview and during the pre-employment verification process. Duration, Location, and Working Pattern:

  • Permanent and Fixed-Term opportunities available, full time or part time hours
  • You will be based at Queen Elizabeth University Hospital / Home First Response Service (HFRS)
  • You will work these hours 5 days over 7, including weekends and public holidays as required by the service.
  • Please note the salary is Pro Rata for part time hours.

Key Responsibilities

You will undertake comprehensive frailty assessments for older adults, support clinical decision-making and the development of management plans, and facilitate safe discharge and admission avoidance where appropriate.

They will work collaboratively with acute, community and social care partners, coordinate referrals and onward care arrangements, and provide leadership and expert clinical advice within the multidisciplinary team.

You will also contribute to service development and quality improvement initiatives and support the delivery of high-quality, person-centred care for older people with frailty and complex needs.

Knowledge, training, qualifications and/or experience required to do the job:

  • Relevant postgraduate qualification in advanced clinical practice, frailty or clinical assessment.
  • Registered healthcare professional, such as a Nurse, Allied Health Professional or Paramedic, with appropriate professional registration.
  • Substantial post-registration experience.
  • Experience of working with older people, ideally within older people's services, urgent care, emergency care, acute medicine, Hospital at Home or community services.
  • Excellent clinical assessment and decision-making skills.
  • Experience of managing patients with frailty and complex needs.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Strong communication, leadership and organisational skills.
  • Knowledge and understanding of person-centred care for older adults.
  • Commitment to delivering high-quality care and supporting admission avoidance and safe discharge.
  • Ability to work collaboratively across health and social care settings.
