Job Description

The Role

Join NHS Greater Glasgow and Clyde as a District Nurse (Specialist Practitioner) and play a pivotal leadership role within our integrated community teams. You will deliver safe, effective, person-centred care while supporting patients to remain independent in their own homes. Working collaboratively across health and social care, you will influence and shape high-quality care pathways for individuals with complex and long-term needs. All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:

  • You will be based at East Dunbartonshire HSCP
  • Working 5 days over 7, including day shift and back shift
  • Full Time, Permanent

Key Responsibilities

  • Lead and manage a district nursing team, ensuring safe, effective and person-centred care delivery across a defined caseload
  • Act as the clinical expert, undertaking advanced assessments, care planning and prescribing within the scope of practice
  • Coordinate multidisciplinary working with health and social care partners to deliver integrated care and support patient independence
  • Manage complex patient needs, including long-term conditions, palliative and end-of-life care, and urgent interventions
  • Provide leadership, supervision, and professional development support to nursing staff and students
  • Monitor clinical quality, lead audits, and promote evidence-based practice to continuously improve care outcomes
  • Respond to changing service demands, prioritising workload and implementing contingency plans to maintain safe care delivery

Knowledge, training, qualifications and/or experience required to do the job:

  • Current NMC registration as a Registered Nurse
  • SPQ in Advancing Practice in District Nursing at Masters Level
  • V300 Independent Prescribing qualification with advanced clinical assessment skills
  • Demonstrable leadership and management experience within a community or primary care setting
  • Evidence of ongoing professional development and commitment to continuous learning
  • Strong communication, decision-making and problem-solving skills with the ability to work autonomously
  • Competence in IT systems and ability to work effectively within multidisciplinary teams
