Job Description

The Role

Skilled Worker visa and Health and Care Worker visa
Duration, Location, and Working Pattern:
The main role of the Health Visitor is to act as the designated named Health Visitor for all children within an allocated caseload from antenatal stage through to school entry age, excluding the unborn child.
You will be responsible for commencing and coordinating the child’s planning process, ensuring the wellbeing and developmental needs of children are assessed, monitored and reviewed in line with national and local guidance.
You will work in partnership with children, families and a range of multi-agency professionals to identify needs and develop effective child-centred plans of care and support. This will involve ongoing assessment, monitoring and evaluation of outcomes to ensure the best possible support is provided to children and families.

- These hours will be worked between Monday and Friday
- You will be based at Rowan Business Park
- Please note the salary is Pro Rata for part time hours.
